Designing a Life of Choice, Connection, and Belonging
At Transition Life Care, we believe that your home should reflect your needs, values, and lifestyle. Our Independent Living Options (ILO) support you to create a living arrangement that works for you — whether that means living on your own, with friends, or with a supportive host family.
While we do not provide the accommodation directly, we walk alongside you in developing and implementing a tailored ILO arrangement that supports your independence, preferences, and goals.

What is ILO?
Independent Living Options (ILO) is an NDIS support that funds flexible and highly individualised living arrangements for people with a disability. ILO is about how you want to live, not where you live — and it allows you to choose the type of home and support model that suits your lifestyle.
ILO is an alternative to more structured support models like group homes or SIL (Supported Independent Living) and is focused on increasing autonomy, community connection, and informal support networks.

Key Features of ILO
- Choice and Control: You decide who you live with, where you live, and what your day-to-day support looks like.
- Flexible Support: Supports are built around you, not a fixed model.
- Community Connection: ILO encourages strong ties to family, friends, neighbours, and your local community.
- Tailored Planning: Every ILO arrangement is unique and developed in two stages — Exploration & Design, followed by Implementation.

How Transition Life Care Supports ILO
While we do not provide ILO housing ourselves, we offer a comprehensive support service to help you explore, design, and implement an ILO arrangement that fits your life. Our role includes:
- Supporting you through the Exploration and Design stage, helping you identify what living independently looks like for you
- Working closely with your NDIS planner, support coordinator, and family to develop a viable ILO proposal
- Linking you with host arrangements, housemates, or tailored community supports through our trusted partners
- Facilitating ongoing support adjustments to ensure your ILO remains sustainable and empowering
- Advocating for your choices while ensuring safeguarding and stability

Who is ILO For?
ILO is ideal for NDIS participants who:
- Want greater independence and to live outside traditional group accommodation settings
- Have informal supports (like friends, family, or a host) that can be built into their lifestyle
- Are seeking a creative and flexible living arrangement
- Have ILO funding in their NDIS plan or are beginning the ILO exploration process
If you’re unsure whether ILO is the right path, we can support you to explore other options and determine what best suits your needs.
